|Summary||Building on the theory and practice based learning of Healthy Communities 1; this course further develops and deepens the students┐ knowledge about the concepts introduced in Healthy Communities 1, such as health and wellbeing of the individual in the context of the family and wider community health. Students are enabled to develop a critical understanding about communities and how communities are engaged and motivated to undertake change to benefit the wider population health beyond that of the individual and the family.
This course will provide students with the opportunity to explore the principal theories, concepts and terminology of sociology relevant to health. Health promotion and education including teaching methods and learning styles that support change management will be developed with the students. Students┐ understanding of social and health legislation which is essential for nursing work in contemporary health and social care environments of today will also be further developed.
This is a core course in year 2 of the Bachelor of Nursing with Honours programme. The course is suitable for health and social care professionals who have undertaken Healthy Communities 1. The course content is delivered through a series of lectures and workshops in weeks 1-6 Semester 2. In addition 1 full study day to take place on a date to be confirmed but likely to be a Monday in early April and students must attend the study day as the 30% assignment is assessed on this day. The focus is on the health of communities as part of a larger society. It builds on the students knowledge and understanding of the impact which community and family dynamics can have on the health of an individual. Utilising taught theories and techniques, students are enabled to develop skills to start to assess and address health and social care needs of communities. The course addresses:
-Assessing the needs of a community
-Community development and enablement
-Health inequalities and social inclusion
-The principles of public health
-To provide students with knowledge about the cultural diversity of communities and the impact this has on health, health needs and the approach taken in health and social care to address health need.
-To promote the students skills and knowledge base in assessing need, planning interventions and understanding approaches to evaluating impact and outcome.

-Feedback on a 1 page plan developed by the student for the oral and visual presentation and submitted via Turnitin
Summative Assessment 30% of the overall course mark
-The assessment takes the form of a 5 minute modified ViPER (Oral and visual) presentation of a poster focused as follows:
-Production of a poster, prepared electronically e.g. in powerpoint, maximum 1000 words, addressing a public health improvement topic identified from the health needs assessment and action plan you are developing for your summative assessment.
Summative Assessment (70% of course mark)
-Produce a community profile and health needs assessment including an action plan, confined to within 2000-2500 words. For the course paper students will use a defined community (for Nursing students this is their year 2 long term conditions community placement).
||Formative feedback is given on a 1 Page plan for oral and visual presentation.
Summative feedback is given via Learn on the presentation and course paper as detailed above.

On completion of this course, the student will be able to:
- Be knowledgeable about the complexity of community
- Identify the important role which health improvement activities have in changing the health of communities
- Have knowledge about the complexities of working with various communities including young people, older people and those who are ethnically diverse
- Be able to assess the health needs of a defined community
- Be able to produce a community profile and an action plan to address the needs of a community, identifying the need, planned intervention, time scale and the proposed evaluation/assessment method to measure outcome
